Livea Medical ("Livea," "we," "our," or "us") is a medical weight loss and metabolic health clinic with eight locations in Minnesota and Wisconsin, as well as a telehealth program serving patients across both states. 3. Telehealth Consent
3.1 Nature of Telehealth Services
Livea offers telehealth consultations via secure video, allowing you to receive medical care remotely without traveling to a clinic. Telehealth services are subject to Minnesota and Wisconsin telehealth regulations and are provided only by Livea providers licensed in those states.
3.2 Acknowledgments for Telehealth Patients
By scheduling and participating in a telehealth appointment with Livea, you acknowledge and agree that:
- You understand that telehealth is not the same as an in-person visit and may have limitations, including the inability to perform a physical examination
- The quality of a telehealth encounter may be affected by your internet connection, hardware, or environment
- In the event of a technical failure during a session, your provider may contact you by phone or reschedule your appointment
- You are responsible for ensuring you are in a private location during your telehealth visit to protect your own privacy
- You are located within the State of Minnesota or Wisconsin at the time of your telehealth visit, or have otherwise confirmed eligibility with Livea staff
- Your clinical information from telehealth visits is stored and handled in accordance with HIPAA and Livea's Privacy Policy
- Telehealth care may not be appropriate for all conditions; your provider will advise you if an in-person visit is medically necessary
3.3 Full Telehealth Consent Form
A complete Informed Consent for Telehealth Services will be presented to you during the intake process prior to your first telehealth visit. That document governs your telehealth care in detail and is incorporated into these Terms by reference.
4. Appointments, Booking, and Cancellation
4.1 Booking an Appointment
You may book a consultation or appointment through the online booking tool on the Site, by phone at 763-319-2020, or in person at any Livea clinic location. Booking an appointment is subject to provider availability and does not guarantee a specific provider or outcome.
4.2 Cancellation and No-Show Policy
We understand that schedules change. Our current cancellation and no-show policy is detailed during the booking process and in your patient intake documentation. 4.3 Rescheduling
To reschedule an appointment, please contact us at least 24 hours in advance by calling 763-319-2020 or using the online portal. We will make reasonable efforts to accommodate your request, subject to availability.
5. Payments and Fees
Fees for Livea services will be quoted to you prior to the commencement of care and are detailed in your patient agreement. Payment is due at the time of service unless alternative arrangements have been made. Livea accepts major credit cards, HSA/FSA cards, and cash. Insurance billing is available for eligible services; however, Livea does not guarantee insurance coverage or reimbursement.
All fees are subject to change. Updated pricing will be communicated in advance. Any disputes regarding charges must be raised within 30 days of the billing date by contacting 763-319-2020.
6. Intellectual Property
All content on the Site, including text, images, graphics, logos, video, and software, is owned by or licensed to Livea and is protected by United States and international copyright, trademark, and other intellectual property laws. You may view, print, or download content from the Site for your personal, non-commercial use only. You may not reproduce, distribute, publicly display, modify, create derivative works from, or otherwise exploit any content from the Site without Livea's prior written permission.
The Livea name, logo, and taglines are trademarks of Livea Medical. Other trademarks and service marks appearing on the Site are the property of their respective owners.

LIVEA'S TOTAL AGGREGATE LIABILITY TO YOU FOR ALL CLAIMS ARISING FROM OR RELATED TO THE SITE OR THESE TERMS, WHETHER IN CONTRACT, TORT, OR OTHERWISE, SHALL NOT EXCEED THE GREATER OF (A) ONE HUNDRED DOLLARS ($100) OR (B) THE AMOUNT YOU PAID TO LIVEA IN THE 30 DAYS PRECEDING THE EVENT GIVING RISE TO THE CLAIM.
Some states do not allow the exclusion or limitation of certain warranties or liability, so the above limitations may not apply to you in full.
10. Indemnification
You agree to defend, indemnify, and hold harmless Livea and its officers, directors, employees, and agents from and against any claims, liabilities, damages, losses, and expenses (including reasonable attorneys' fees) arising out of or related to your use of the Site or Services, your violation of these Terms, or your violation of any rights of another party.
11. Medical Malpractice and Clinical Care
Nothing in these Terms of Service limits or waives any rights you may have under Minnesota or Wisconsin medical malpractice law with respect to clinical care provided to you by licensed Livea providers. These Terms govern your use of the Site and digital Services; they do not govern the standard of clinical care.
12. Governing Law and Dispute Resolution

These Terms are governed by and construed in accordance with the laws of the State of Minnesota, without regard to its conflict-of-law principles.
12.2 Dispute Resolution
Before initiating any legal proceeding, you agree to contact Livea and attempt to resolve any dispute informally by calling 763-319-2020 or writing to us at [CLINIC ADDRESS]. If informal resolution is unsuccessful within 30 days, either party may pursue available legal remedies.
12.3 Venue
Any legal proceeding that cannot be resolved informally shall be brought exclusively in the state or federal courts located in Hennepin County, Minnesota. You irrevocably consent to the personal jurisdiction of those courts.
13. Changes to These Terms
We may update these Terms at any time by posting the revised version on this page with an updated effective date. Your continued use of the Site or Services after the revised Terms are posted constitutes your acceptance of the changes. We encourage you to review these Terms periodically.
